Correspondence regarding your bill should be mailed to the above address.
It is important to remember that your bill covers services provided by the surgery center: room, nursing care, equipment and supplies, housekeeping and linen. It may or may not include services ordered by your physician: pathology, x-rays and laboratory tests. The bill does not include charges for your personal physician, surgeon, anesthesiologist, pathologist, or radiologist. You will receive separate bills from these physicians.
